Insight, state set to meet Tuesday

Insight Health System officials are scheduled to meet Tuesday with the Ohio Department of Health in an effort to prevent the state from pulling its license to operate Insight Hospital & Medical Center Trumbull in Warren.

An agency spokesman said Insight requested the hearing in response to its Nov. 17 revocation notice. In the letter, the state said it would pull Insight’s license based on violations its inspectors uncovered dating back to March 27.

The Akron law firm of Brennan Manna Diamond, representing Insight, asked for a hearing on Nov. 26. The state informed Insight it may present evidence and examine witnesses.

“You also may present your position, contentions and arguments in writing,” the revocation notice said.

The session will not be open to the public.

The action to revoke Insight’s license followed a facility inspection Nov. 12.

First, “the facility failed to maintain comfortable temperatures in the emergency department waiting room,” the agency said.

The inspection also found Insight failed to integrate its emergency services “with other departments of the hospital in such a way that the hospital can immediately make available the full extent of its patient care resources to assess and render appropriate care for an emergency patient.”

The report was critical of Insight’s handling of a stroke patient, who was not administered intravenous thrombolytics, also known as clot-busting medication.

“Instead that patient waited for two hours in the emergency room before being transferred to another hospital for proper care, jeopardizing the patient’s ability to receive these medications within the narrow window of time available for their administration,” the agency report said.

Insight has closed its Trumbull County properties twice in 2025. Citing financial pressures caused by former owner Steward Health Care, Insight stopped all operations March 27, including Insight Rehabilitation Hospital Hillside, Howland.

Insight Trumbull reopened Oct. 14 when the Ohio Department of Health gave its approval that morning. However, the Centers for Medicare & Medicaid Services revoked its certification of Insight on Oct. 10. CMS provides coverage reimbursement to facilities serving Medicare and Medicaid patients.

In response to the state’s revocation letter, Insight Trumbull closed its doors Nov. 26.

“Because of the department’s inaccurate accusations and setting back the reopening process, the hospital is not able to function in a sustainable manner,” Insight said at the time.

The agency said it could have, but did not, close Insight earlier.

“Additionally, the Ohio Department of Health did not take licensure action during the temporary closure of Insight Foundation of Trumbull and allowed the hospital multiple opportunities to become fully operational and ensure compliance,” the agency said.

It noted uncorrected violations reported on March 27, Sept. 23, Oct. 9 and Oct. 22. The areas covered delayed maintenance, life safety, water management, patient care, and heat and boiler issues.

Insight officials did not respond to an email Thursday asking if they intended to attend Tuesday’s meeting.
