Probe begins in Kinsman fatality

KINSMAN -- An investigation is underway by the Trumbull County Sheriff's Office and the U.S. Department of Labor's Occupational Safety and Health Administration after a workplace incident at McGill Septic Tank Company in Kinsman resulted in the death of 37-year-old Jennifer Plant.

According to county Sheriff Paul Monore, Plant was pinned between a forklift and a truck Friday afternoon while workers were loading products at the company's premises. Emergency services responded to the scene and transported Plant to the hospital, where she died from her injuries.

Monore said that while the circumstances were horrific, there's no evidence to suggest foul play. He stated, "When there's a fatality, OSHA jumps in with both feet and they look at everything, all the employees that are involved, who was operating the equipment, they investigate from top to bottom."

A representative from OSHA provided insight into a previous inspection at McGill Septic Tank Company in 2013. During that inspection, the spokesperson explained the company faced two serious violations and two other-than-serious violations.

Following an informal conference, McGill Septic Tank Company paid $2,420 in penalties. The violations included issues such as debris on the floor, electrical strain on cords, failure to conduct periodic inspections of energy control procedures and failure to certify powered industrial vehicle operators every three years, as required.

OSHA has opened an inspection into McGill Septic Tank Company regarding the recent fatality. They have six months to complete their investigation, during which they will scrutinize workplace safety and health regulations. If violations are found, citations and monetary penalties could be proposed.

McGill Septic Tank Company, located at 8913 Williamsfield Kinsman Road, has not provided a statement regarding the death.
