WARREN - Rhonda Jean Creak, 49, died Wednesday, Sept. 19, 2012, at her home of a heart attack.
She was born May 1, 1963, in Warren, the daughter of Almonta Ware Creak and George Creak.
She is survived by her mother, Almonta Ware Creak, with whom she lived.

She was preceded in death by her father and a brother.
There will not be any services or visitation. Cremation will take place.
